Required Document List

1. A completion certificate or its substitute for the building that is to be licensed

2. Building completion certificate for the house or an approved plan indicating the exact location for practicing the activity

3. A copy of the Electricity and Water Certificate (KAHRAMAA)

4. A picture of my home address plate (Enwani)

5. Model Photo montage panel brand name on the front and specified the required sizes so that the rise of no more than 120 cm

6. Ownership deed

7. Copy of the QID of the owner of the property

8. A letter of no objection from the owner

9. The rental agreement form or its substitute (deposit receipt at the Dispute Settlement Committee or a court or what serves in the opening of a disputed file)    

10. Declaration form for authorization to rent real estate (sub-leasing)

11. A letter of no objection from the guardian and a copy of the ID card

12. Copy of the license holder's health certificate

13. A copy of the professional registration record
